People frequently get painless little bumps under the skin, called cysts. Skin cysts, also known as sebaceous cysts, can develop for many reasons — and sometimes for no obvious reason.

Skin cysts are benign, or noncancerous, and they usually do not cause problems. They’re usually painless and nothing to worry about, unless they get sore or infected.

Causes of a sebaceous cyst

Sebaceous cysts form due to damaged or blocked sebaceous gland duct or due misshapen or deformed duct at birth.

Genetic conditions, such as Gardner’s syndrome  or basal nevus syndrome leads to a sebaceous cyst.

Sebaceous cyst occurs due to trauma to the area, trauma may be scratch, surgical wound, or acne

Hereditary conditions like steatocystoma multiplex may also lead to sebaceous cyst.

Most of the time sebaceous cysts usually referred as epidermoid cysts or pilar cysts.

  • An epidermoid cystoriginates from the epidermis and it contains keratin, which is the protein in the top layer of skin. However when the cells move into your skin instead of shedding they result in the formation of epidermoid cysts.
  • A Pilar cystoriginates from the hair follicles and also contains keratin.

Appearance of sebaceous cyst

Sebaceous cysts are small, round bump or a lump under the skin generally found on the face, neck, back and abdomen. Cysts may range from 1cm (pea-size) to 5 centimetres.

Sebaceous cysts and on physical examination feels harder than normal skin, a spongy sensation at pressing on it. A tiny blackhead plugging the central opening of the cyst called punctum.The cyst contains mixed fluids of thick, yellow, smelly material and white flakes of keratin.

If the cyst is more 5 centimetres in diameter and showing signs of infection, such as redness, pain, or pus drainage it is necessary to consult a doctor.

Cyst Removal Treatment

Skin cysts don’t need treatment most of the time. They are usually non-cancerous, painless and nothing to worry about unless they get sore or infected. Since most cysts aren’t harmful to your health, you can leave a cyst alone if it doesn’t cause discomfort or cosmetic problems.

It’s important to note that without surgical removal, your cyst will usually reoccur. Our best advice is to remove completely through surgery.

Medication

Medication is useful to treat only infected sebaceous cysts. Oral or topical or injectables like antibiotics & steroids are used to reduce the inflammation and pain of the cysts. 

Incision and drainage (I&D)

In this method a small cut is done and gently squeezed out the content. Though the procedure is quick and easy but often reoccurs after the treatment.

Excision

There are two types of excision conventional wide excision and minimal excision.

Conventional wide excision

Dermatologists completely remove the cyst wall or sac from the surrounding tissue to prevent the recurrence. But the procedure requires stitches and leaves a scar.

Minimal excision

In this procedure dermatologists make 2-3mm of cut to remove contents and cyst wall completely, which causes minimal scarring and heals naturally.

After cyst removal, dermatologist prescribes medication of antibiotic pills and ointment for healing process. You may also, prescribed with anti scar cream to reduce the appearance of surgical scar.
